	Effective April 30, 2012, Chinese Consulates will enforce form jurisdiction requirement. You must use correction version of application form(s) based on where you reside. Your application will be delayed or rejected if you use the wrong version:
	Use V2011 New York Version, if you reside in following states:
	if you reside in following state: Connecticut, Maine, Massachusetts, New Hampshire, New Jersey, New York, Ohio, Pennsylvania, Rhode Island and Vermont.
	Use V2011 DC Version, if you reside in following states:
	Washington DC, Delaware, Idaho, Kentucky, Maryland, Montana, Nebraska, North Carolina, North Dakota, South Carolina, South Dakota, Tennessee, Utah, Virginia, West Virginia, Wyoming Colorado, Illinois, Indiana, Iowa, Kansas, Michigan Minnesota, Missouri, Wisconsin Alabama, Arkansas, Florida, Georgia, Louisiana, Mississippi Oklahoma, Texas Arizona, Hawaii, New Mexico, Southern California, Pacific Islands Alaska, Nevada, Northern California, Oregon, Washington

	Fill out the application form completely. Incomplete form will be rejected. All filed's are mandatory. You may use "None" or "N/A" for field that doesn't apply to you. Please fill out the application form on your computer, then print a hard copy and sign. Hand written form is no longer acceptable. Change or correction is not allowed on the printed form. You must print a new form in case of any change or correction. Please also submit the application electronically by email by click the submit button. We suggest you to save a copy of the application form on your computer once completed in case of change or modification. The form must be printed on white letter size paper with black ink or toner. Thermal paper, color paper or paper with holes or perforations are not acceptable. The application form here is only for US passport holders and US residents.

	V2011B is a Supplementary Visa Application Form. Only required for people applying for Work Visa, Student Visa or someone applying for a visa in a country or territory other than the country of your nationality
